how do you clean substrate liek that picture?

trumpet snails, and tiny immersible gardeners with scuba tanks.

Training the snails to do your bidding is harder, while the gardenrs are more expensive.

Simple answer:  You don't

If the substrate is has a thick ground cover like that, you just hover your gravel vac as close as you can.  The only stuff that will collect under there is mulm, and mulm is good for a planted tank.
