My betta fish

My betta fish is really sick & I'm not sure what to do. The pet store told me it was bacterial so I've been using melafix & primafix but it doesn't seem to be doing anything. He just floats at the top most of the time & when he does swim it looks like he's really struggling. Could someone help me identify the problem

keep your water really clean.

Is he eating?

I'd do daily water tests, and make a plan of action from there.

I've heard that Indian Almond Leafs are good to use for this.
