Holes in leaves

I have a problem keeping baby tears (HM). After a few days in my tank I get holes in the leaves and the leaves turn slowly transparent. I am dosing 3x/week with potassium nitrate and potassium phosphate plus 2x/week micro nutrients.

What is missing? Thanks!

Could you give us a little more to go on? Did you buy them growing emersed? What is your dosing regime? Your substrate? Lighting? Water source? Are you CO2 dosing? etc
